1. Your care plan

When you first meet the staff from your social care service they should spend time talking to you about what you need.

The staff that will care for you are called care workers.

You and your care workers should agree on a care plan together.

A care plan is a document that talks about:

  • All your needs. These include your social, personal and health needs
  • How these needs will be met

You and your care workers should keep checking the plan. This is to make sure the plan is working well and is still right for your needs.

Your care workers should also keep asking you if you are happy with the care that you get.

Remember: It is very important that you tell your care workers what your needs are. They will only know how to help you if you tell them what your needs are.
